Not everyone struggling with their weight needs medication, and not everyone who wants it will be prescribed it. This article is about everything else, and it is useful to a reader who will never pay us anything.
Why diets fail
Not because people are lazy. When energy intake drops, the body responds by spending less and making you hungrier. That is physiology, not weak will. It is why most people lose weight and are back within two years. Anything you cannot do for years ends exactly this way.

What has the best evidence behind it
- A way of eating you can actually keep up. The specific approach (lower carbohydrate, Mediterranean, counting calories) matters less than whether you will still be doing it in a year.
- Enough protein, and resistance training. Losing weight costs you muscle as well as fat; this is how you lose the least of it.
- Sleep. Too little raises hunger and worsens food decisions. It is one of the most underrated factors there is.
- Measuring regularly. People who weigh themselves and record it hold onto the result more often.
- weight loss was enough to cut liver fat and improve insulin sensitivity
- 5%
- less severe sleep apnoea went with a 10% weight loss
- 26%
- after a diet began, appetite hormones had still not returned to where they started
- 62 weeks
- more hunger after two nights of short sleep, in a small trial of 12 young men
- 24%

What does not work, whoever is promising it
- Detoxes and teas. Your kidneys and liver do that job, not a boxed programme.
- Fat burners bought online with no ingredient list and no approval.
- Extreme short diets before an event: they come back with interest.
That is physiology, not weak will.
When it becomes worth seeing a doctor
When you have tried repeatedly and it does not hold, when you have conditions connected to weight (high blood pressure, diabetes, sleep apnoea), or when your weight is getting in the way of ordinary life. That is not an admission of failure. It is the point at which this stops being about discipline and starts being about medicine.
Among other things, a doctor checks whether something else is driving it: thyroid function, medicines you take for something unrelated, hormonal causes. That is not something you can rule out yourself.
a.clinic is a technology platform. Healthcare is provided by a licensed doctor, who decides whether any treatment is appropriate for you. We do not decide about specific medicines and do not write about them.
- Do I have to try losing weight on my own first?
- It is not a requirement, but a doctor will want to know what you have already tried and how it went. It is part of the picture they decide from.
- Is losing weight without medication possible?
- Yes, and for many people it is the right path. Keeping the result tends to be harder than achieving it, and that is the difference between a diet and a change that lasts.
- How much is it healthy to lose in a month?
- A slower rate holds better than a fast one. What rate suits you depends on where you are starting and on your health, which is a question for a doctor rather than an article.
